Demi More was hospitalized mon monday night,she was suffering from substance abuse..Paramedics responded to a 911 call placed 10:45pm PT and assessing her condition transported her from home Los Angeles to a local hospital...

She has reportedly to have moved to a facility to seek treatment for substance abuse , sources told the entertainment website,....

"Because of the stress in her life right now,Demi has chosen to seek professional assistance to treat her exhaustion and improve her overall health" a rep of the actress said

"She looks foward to getting well and is grateful for the support from family and friends" her rep added

In November,Moore 49 announced ending her marriage to Ashton Kutcher , after 6years of amid reports Kutcher 33 had engage in multiple extramarital affairs...
